2022  ISCD/IOF國際骨密認證課程及考試

Technologist Course

報名簡章

一、 報名日期: 111.03.16至111.04.11，逾期恕不受理。活動當天恕不受理現         

              場報名繳費。(此課程僅限放射師報名)

二、 上課日期: 111.04.16(六) 08:00-18:00; 111.04.17(日) 08:00-12:30

三、 上課地點: 台北榮民總醫院 致德樓第五會議室

              (臺北市北投區石牌路二段201號)

四、 報名方式: 分為2階段。2階段皆須完成報名才算報名成功。

1. 階段一: 請至IOF網站報名並以信用卡繳交IOF考試費(Attestation Exam) 50美元，請截圖刷卡報名成功之頁面，於第二階段報名時上傳。(此為IOF規定必要之費用)(TOA會員也需繳費)

2. 階段二: 請至學會網站報名http://www.toa1997.org.tw。報名成功會

       收到系統的email回函，若沒收到請洽詢秘書處02-23143686，

       toaservice@gmail.com。

         請注意: 非會員請先繳費，保留繳費證明(報名時需上傳)，再上網登入

               登打報名資料。

五、 報名費用(含TOA考試費用): 如下表，需於報名截止日前繳費。

凡具會員資格，已繳清會費，全程參與課程者，由學會補助全額報名費(無須預繳)。欲申請加入會員者請於111.04.06前上網申請入會並完成繳費，再以會員身分報名此課程，請務必先申請入會再報名課程。

六、 有關入會申請相關注意事項: ​

50. 若您曾經參加過本會舉辦之課程，已申請過非會員者，請先email至秘書處(toaservice@gmail.com)告知欲申請入會，刪除您的非會員資料後，再進行入會申請程序。
50. 欲申請入會之學員，請先繳費再填寫入會申請資料，需上傳繳費證明單據，以利核對匯款者之帳號資料。http://www.toa1997.org.tw/member/signup_info.php
50. 送出申請成功會收到系統email回函告知已送出申請成功，若未收到回函，請來電或email洽詢秘書處確認。
50. 入會申請會先就送審資料初審，初審通過系統會email發送已入會成功之通知函，收到此通知函後再以會員身分登入報名課程。

七、 本課程及考試退費辦法(僅適用於TOA之非會員繳費)

學員報名本會(TOA)認證課程並完成繳費後，不得展延，但得依本辦法申請退費。

1. 於報名截止日前(含)，得申請退還全額認證課程費與考試報名費，惟須扣除

匯款手續費。

2. 於報名截止日後(不含)，不得申請退還認證課程與考試報名費。

3. 認證課程當日如因颱風、地震、空襲、水災、火災等不可抗力之重大偶發或

    突發事件，臨時無法赴試場參加者，得申請退還全額報名費，惟須扣除匯款

手續費。

八、 REFUND POLICY(適用於IOF之繳費)

a.       Individual Name Changes (Substitution of Registration)

• An administrative fee of 30% of the registration rate will be charged for a name change. Written permission from the original participant will be required, along with full contact details of the substitution.

b.       Individual Cancellation & Refund Policies

• Notification of cancellation and refund requests must be submitted in writing by e-mail to the course organizer.

• Participants who submit a refund request more than 48hrs before the course begins, are entitled to a 50% reimbursement. No refunds will be given to participants who cancel less than 48hrs before the course begins.

• All reimbursement will occur after the course.

c.       Cancellation or Modification of the Course

• In case the LOC decides to alter or cancel the course for a reason that is not due to a Force Majeure, 100% of the registration rate will be reimbursed for participants that can no longer attend the course.

• Should the LOC decided to cancel the course, the LOC will need to notify IOF, ISCD and all participants as to why the course has been cancelled.

• The LOC cannot alter or cancel the course less than one month before the course date.

• Participants will receive a reimbursement during the 30 days after the cancellation.

d.       Cancellation or Modification of the Course due to Force Majeure

• In case of force majeure, the LOC has the right to alter or cancel the course without prior notice, however a notice of the occurrence shall be given by the LOC as soon as reasonably possible.

• Force majeure shall mean any circumstance beyond the reasonable control of the Organizer which prevents or impedes the holding of the course, including, but not limited to, government action, war or hostilities, riot or civil commotion, plague or other epidemic such as SARS, bird flu, etc., earthquake, flood, hurricane, cyclone, fire or other natural physical disaster, explosion, accident or breakdown, strike, lack of the usual means of transportation or terrorism or due to events which are not attributable to wrongful intent or gross negligence of the LOC.

• The LOC shall not be liable for any direct or indirect, incidental or consequential damages, losses, expenditures or any other inconveniences or costs caused by such modification or cancellation of the course.

• Furthermore, registration fees will not be reimbursed by the LOC.

九、 其他注意事項：

1. 報到時請出示身分證+Covid-19疫苗黃卡(須有至少2次接種)或健保卡。完成報到程序後提供上課講義。
2. 本考試無題庫。考試嚴格禁止作弊，一經發現作弊，則取消考試資格，並且不得申請退費。手機請放入包包，嚴格禁止拍攝考題。
3. 若您有事必須先離開不能參加考試，請提前告知主辦單位，但不得申請考試延後或提早考試。
4. 考試結果於考試結束2週內公告於學會網站，4週內寄送考試通過證明電子檔。如有需要紙本證書，請自行下載列印，本會恕不受理申請寄送紙本證書。 
5. 停車：原則不提供。當日若有提供院區內停車會於汽車入口處與學會網站公告(www.toa1997.org.tw)。
6. 本會鼓勵環保，會場有飲水機，請學員們自備水壺或保溫瓶，原則上不提供紙杯。
7. 此課程提供免費餐食，請於報名時正確勾選所需餐食種類(葷食、素食、不用餐)。
8. 會場的冷氣空調原則上已依標準設定，不得依個人需求調整，請斟酌個人身體狀況攜帶外套或保暖衣物備用。
9. 因應Covid-19疫情，出席者請全程配戴口罩，且確認告知本會14天內之接觸史及旅遊史。若您當天身體感到不適，請勿到會場，請您以email告知主辦單位，可於會後辦理全額退費。與會者請務必盡告知責任。
10. 會員免報名費為會員之福利，若因故無法參與課程，請務必於三天前通知主辦單位，以利取消您的餐點以及講義之印刷，避免造成浪費。無故不到者下次報名須繳交報名費。
